Medicaid Support Programs

For individuals enrolled in Washington Apple Health (Medicaid) who need long-term support with employment or housing. These services are not time-limited. We stay with you for as long as you need us.

What is Supported Employment?

Supported Employment is long-term, individualized help getting and keeping a job. Unlike time-limited employment services, this program stays with you for as long as you need it. We help with everything from figuring out what kind of work you want to do, to finding the right job, to coaching you through your first months on the job and beyond.

Who is this for?

  • Individuals enrolled in Washington Apple Health (Medicaid) who want to work but face significant barriers
  • People with mental health conditions, substance use disorders, or physical disabilities that make it hard to get or keep a job
  • Individuals who have been fired, let go, or unable to hold a job due to their disability
  • People currently receiving state disability benefits (HEN or ABD) who want to explore employment
  • Anyone who has tried working before but could not sustain it without ongoing support

Discovery

We start by getting to know you. What kind of work interests you? What has worked or not worked in the past? What does your ideal day look like? We use this to build a realistic employment plan.

Job Development

We go out and find opportunities that match your goals. This includes reaching out to employers, identifying openings, and sometimes creating custom positions that play to your strengths.

Job Search Support

We help you with applications, resumes, interview prep, and anything else you need to land the right position. We do this alongside you, not for you.

On-the-Job Coaching

Once you are hired, we provide hands-on coaching at your workplace. We help you learn your tasks, navigate coworker dynamics, and communicate with your supervisor.

Follow-Along Support

After you settle in, we do not disappear. We check in regularly, troubleshoot problems, and provide support for as long as you need it. There is no time limit on this service.

Job Placement through our Employment Services is a shorter-term service focused on getting you hired and through your first day. Supported Employment is a longer-term program that includes everything from career exploration to ongoing coaching after you are hired. There is no 90-day cutoff. Supportive Housing helps you find a place to live, get settled, and stay stable long-term. Whether you are currently without housing, at risk of losing your home, or transitioning out of a facility, we provide hands-on support with everything from apartment searches to lease negotiations to connecting you with community resources. This service is not time-limited.

  • Individuals who are currently homeless or living in an unstable housing situation
  • People at risk of losing their housing due to health, disability, or financial barriers
  • Individuals transitioning out of a hospital, psychiatric facility, correctional facility, or residential treatment program
  • People enrolled in Washington Apple Health (Medicaid) with a documented health or disability need
  • Anyone who has cycled through multiple housing situations and needs long-term stability support

Housing Needs Assessment

We start by understanding your current situation. Where are you now? What are your barriers? What kind of housing makes sense for your needs and budget?

Housing Search

We help you find available units, fill out applications, gather required documents, and navigate waitlists. We do this with you, not behind the scenes.

Move-In Support

Once you are approved, we help with lease review, move-in logistics, budgeting for deposits and first month costs, and connecting you with furniture or household resources if needed.

Landlord Coordination

We serve as a point of contact between you and your landlord when needed. If issues come up around rent, repairs, or lease terms, we help mediate and resolve them.

Ongoing Stability Support

After you are housed, we continue checking in. We help with budgeting, connecting you to healthcare and mental health services, crisis planning, and building the daily skills that keep you housed long-term. There is no time limit.
